Last year of school: a simple checklist

Talk

  • Ask your young adult what they want after school, in their own words.
  • Talk to the school about what transition help they offer.
  • Tell your Support Coordinator or NDIS contact that school is ending, so the plan can match.

Plan one thing for each week

  • Pick one regular activity that will keep going after school. For example, a course, a volunteer shift or a sport.
  • Visit the place before school ends.
  • Practise the trip there, a few times, before it matters.

Get the paperwork ready

  • Their own bank account and card.
  • ID documents they can use, like a photo card.
  • A phone with important numbers saved.
  • Any travel concession they can get. Ask Transport for NSW.

Practise everyday things

  • Getting up and ready without being reminded every step.
  • Buying lunch and checking the change.
  • What to do if a bus is late or a plan changes.

Pick one item to start this week. Small and steady works better than doing everything at once.
