What this site is for

General information about our services, and forms to contact us, take the Independence Scorecard or make a referral. Using the site does not create a service agreement.

The Independence Scorecard

The Scorecard is an informal, self-reported tool. It is not a clinical assessment, a Functional Capacity Assessment, an NDIS eligibility or funding assessment, or a diagnostic instrument, and it must not be used as evidence in a plan review or funding decision. Results describe answers given on one day by one person.

Content on this site is general in nature. It is not medical, allied health, legal or financial advice, and it does not replace advice from a qualified professional who knows the person.

Funding

Waymaker Pathways is not a registered NDIS provider and works with self-managed and plan-managed participants. Whether a particular support can be funded depends on the participant’s own plan and circumstances.

Acceptable use

Please do not submit false information, other people’s personal information without authority, or automated submissions. We rate-limit forms and may block abusive traffic.

We try to keep the site available and accurate but do not guarantee either. External links are provided for convenience; we are not responsible for their content.
