Privacy collection notice

Independence Scorecard

• Collected: first name, email, twelve answers, selected priority areas, your main question.
• Purpose: to send your results and to make a first conversation useful.
• Not collected: surname, phone, NDIS number, diagnosis, medical information.
• Consequence of not providing it: you can still take the Scorecard, but we cannot email you a copy.

Enquiry / Independence Call

• Collected: name, email, optional phone, your role, reason for contact, preferred contact method, optional message.
• Purpose: to reply to you and work out whether Waymaker is a fit.
• Marketing: none. Sending an enquiry does not subscribe you to anything.

Referral form

• Collected from you: name, organisation, role, email, phone.
• Collected about the young adult: first name only, age band, suburb, funding management type, broad goal areas, short non-clinical support summary.
• Purpose: to assess preliminary fit and contact you.
• Consent: you confirm you have authority or consent to share this. That confirmation and its wording version are recorded with the referral.
• Later: if the referral progresses, further information is collected directly with the participant and family through a secure intake step - not through this website.
